A Ho’okele Health Navigator
will listen and work with you and your family
to define your eldercare needs and support
you in making the right choices for your
family. Whether your goal is to maintain
an elder at home or to explore options at
assisted living facilities or any combination
in between, Ho’okele can help:

Develop a Plan. A
Health Navigator will develop
a customized Plan for your elder and
can even help to make appointments and
accompany you to visit various resources
in the community that may suit your
plans.

Evaluate Cost Options.
Ho’okele can summarize your costs
of different care options for your elder
so you can make an informed decision
about the right settings of care for
your family.

Implement the Plan,
if you choose. A Health Navigator
can handle all the details for you including
finding and engaging the service providers,
facilities, equipment, medical care,
etc.

Independently monitor
of your elder’s condition and
service providers. We can oversee and
monitor the service providers and intervene
if necessary to take corrective action.
We provide regular reports to family
members via regular conference calls
and through our web-based Personal
Health Record.

Give peace of mind
to families who live long distances
away. We can bridge the geographic gap
by serving as a surrogate family member
through our Family
Liaison services and be your eyes
and ears for elders living in our area.

Visit your elder regularly.
Even for elders in an assisted living
facility, a Health Navigator
can visit them, check on their progress,
assess their condition, advocate for
their needs with the facility as necessary
and report back to you so that you have
peace of mind that all is well with
your elder family member.
- Accompany elders to physician
office visits. Our RN’s
will do more than physically be there
with your elder, they will advocate for
their needs. They ask questions, provide
Patient
Advocacy services as well as document
and communicate to you any changes in
health status, medications, regimen, and
anything else that needs to be done through
Personal
Care Coordination services.
