Membership Programs are ideal for clients
who desire ongoing support of a Personal
Health Planner at a consistent and reliable
cost. Membership programs are designed to
meet the differing needs of each individual.
Members benefit from a 10% discount on any
of Ho’okele’s services which
are not included in their selected package.
• Ho’okele Personal
Health Record Membership –
our most basic membership program which
includes a Personal Health Planner to assist
with obtaining your medical records, populate
your web based electronic health record
and keeping the record up to date so that
the most current information is always available
in the event of an emergency. When you need
the services of a Personal Health Planner
in the future, Ho’okele will be familiar
with your medical history to be able to
step in quickly and help.
• Ho’okele Standard
Membership – standard membership
includes a Personal Health Record plus a
designated Personal Health Planner to assess
your particular health care needs, to support
you via telephone whenever you have a questions
or concern, to work with you and your physician
to develop an annual care plan and coordinate
the implementation of the plan with you
through any of Ho’okele’s services
for approximately 4 hours of in-person services
per year.
Your Personal Health Planner will be ready
to assist you and familiar with your personal
and medical history in an emergency or when
you need more intensive navigation services
for example if you are hospitalized or diagnosed
with a serious medical condition.
• Ho’okele Ali’I
Membership – Ali’I
membership includes all the benefits for
Standard Membership plus an expanded 20
hours of in-person Personal Health Planner
services. The Ali’I membership is
ideal for management of eldercare services
on an ongoing, continuous basis or for coordinating
care for long distance caregivers.
