Carrie Kitashima, RN, has
over 15 years of experience in nursing in
acute care, home care, respite care, in-vitro
fertilization, and blood bank. Carrie has
served as a staff nurse at Kapi’olani
Medical Center on a variety of nursing units
including: labor & delivery, post partum,
high risk perinatal care, urgent care, women’s
and pediatric outpatient clinics. She is
a graduate of the University of Hawaii School
of Nursing and has held various supervisory
and management positions most recently as
the Director of Nursing at the Blood Bank
of Hawaii.
Norine Wong, MSW, has
been a Medical Social Worker for over 25 years.
She has extensive experience as a medical social
worker at both Queen's Medical Center and in administrative
roles as Director of Family Services at Shriners Hospital
for Children and Executive Director with Community Case
Management Corp. In her role as Executive Director,
Norine developed the private case management company that
included contracts with DHS, private care management for
the elderly living in communities and consultative services
for acute and long term care facilities, and agencies
managing medically fragile individuals in the community.
She co-authored and implemented a CHOICES grant that
created an inter-agency transition program to assist
youths with disabilities in transitioning towards
independence, and has served in multiple
leadership roles on community based initiatives.
Norine received both her Bachelor's and Master's
degree in social work from the University of
Hawaii at Manoa. She is a licensed social worker and
holds certifications with the Academy of Certified
Social Workers, and is a Certified Case Manager.
Vicki Philben, RN, has
been a Registered Nurse and healthcare
administrator in Hawaii for over 30 years.
Her experience has included Infection
Control, Director of Nursing, a Peace Corp
Medical Officer, and Kuakini Health System
Administrator for Quality and Patient Outcomes.
She received her Bachelors’ of Science degree
from Boston University and Master of Public
Health from the University of Hawaii at Manoa.
She is a past Vice Chair of the Government
Relations Committee for the Healthcare
Association of Hawaii, a member of the
Hawaii Association for Health Care Quality,
and Sigma Theta Tau, Honor Society of Nursing.
Vicki holds certification with the national
Association for Healthcare Quality.
Chris Baxendale, PT, has
practiced physical therapy in Hawaii for
over 25 years in acute care, short term
rehab and home care. Chris is also a certified
lymphedema therapist. In 2004, Chris was
appointed to the Hawaii Board of Physical
Therpay and presently serves as its Vice
Chair.
Hui-I Chen, OT, has practiced
occupational therapy in Hawaii for over
10 years in various healthcare settings.
Hui-I also earned a Master of Public Health
degree from Johns Hopkins University. Hui-I
is currently the Vice President of the Hawaii
Occupational Therapy Association and has
served in this position for the past 4 years.
Jodi Leslie MPH, RD, is a
registered dietitian with over 10 years
experience in nutritional counseling,
education, and research. Her professional
experience includes clinical dietetics
with the Queen's Medical Center, Kona
Community Hospital, and Life Care Center
of Kona and nutrition education and
planning with the Hawaii Department of
Health, Hawaii department of Education
and the University of Hawaii. Jodi has
experience conducting nutrition-related
research, with a current focus on worksite
wellness. She has also provided technical
assistance and consulting to local chefs
and farmers along with the local media
and television productions. Jodi has
a Bachelor of Science in Food Science
and Human Nutrition and a Master in
Public Health with specialization in
Maternal and Child Health and Public
Health Nutrition, both from the University
of Hawaii at Manoa. She is currently
pursuing her doctorate degree in
public health with a focus on chronic
disease and obesity research.
Stephanie Miyasaki, RN,
has been a practicing nurse in Hawaii for
over 25 years in intensive unit care. She
also has additional experience in quality
management, medical staff peer review, telemedicine,
and health informatics. Stephanie is an
active member in the Hawaii Chapter of the
Healthcare Information and Management Systems
Society and served as its Secretary from
1999-2001.
Deanna Moncrief, MS, CNS,
is a certified nutrition specialist with ten
years of experience in nutrition counseling
and employee wellness. Deanna coordinated the
employee wellness program the Queen's Health
Systems before starting her own company in 2002,
now called Benchmark Wellness. Ms. Moncrief
also enjoys a busy private practice working
with people wanting to manage their weight,
prevent or manage diabetes, or older adults
wanting to maintain their independence by
staying healthy. Ms. Moncrief received her
undergraduate degree in Food Science and
Human Nutrition from the University of
Hawaii at Manoa, and her graduate degree
in Nutrition and Clinical Dietetics from
the Chicago Medical School. She is an
active member of the American and Hawaii
Dietetic Associations, the Society for
Nutrition Education, the Food Security
Coalition, and the Hawaii Public Health Association.
Carolyn Schwartz, RN,
has been a Registered Nurse for over 30 years.
Her experience has included Infection Control,
ICU-CCU, ED, OB, and leadership roles in
Perioperative Nursing and Vascular and Urological
specialties. She is a graduate of St. Luke's
Hospital School of Nursing, completed her
Perioperative Nurse Residency at Wyoming Medical
Center, is a Certified Nurse for Operating Room
and is currently enrolled for her BSN.
Stacy Ujimori, RN, has
been a practicing nurse in Hawaii for over
5 years in acute care at Kuakini Medical
Center. She also specializes in case management,
utilization, and quality management and
is a Certified Professional Utilization
Reviewer.
Sue Watts, RN, has been
a practicing nurse in Hawaii and Philadelphia
for over 25 years in acute care and psychiatric
nursing and also has a Masters degree in
organizational management. Sue has worked
in leadership positions in nursing, nursing
education and patient safety within Hawaii's
hospitals and has served on the board of
directors for EduCenter for Nurses.
Sydney Wiecking, has 25
years of experience educating and advising
children, adolescents, and adults. Her
background includes working as a teacher,
counselor and dean at the middle and high
school levels. For the past twelve years
she has run an accredited personal training
business. This led Sydney to complete a
masters-level certification in parent
coaching, and wellness coaching and to
expand her business to work as a Life Coach.
Sydney has several related academic and
professional degrees and accreditations.
She received a BA in Psychology from Hawaii
Loa College in 1981 (now Hawaii Pacific University).
She received her personal training certifications
from the American Council of Exercise (1995) and
the American College of Sports Medicine (2001).
In 2007, she completed two years of academic study
and practicum in parent coaching from the Parent
Coaching Institute (PCI). In 2008, Sydney
completed her training and certification as
a Wellness Coach through Wellcoaches.
