  • Visit you when you are in the hospital to communicate on your behalf with hospital staff about your care and troubleshoot any concerns you may be having.

  • Answer any questions you may have about your treatment, illness or care in partnership with your physician and the hospital staff.

  • Help to coordinate your discharge to your home.
