Navigator will:
- Help you to set up a web-based Ho‘okele Personal Health Record which would allow access to your up-to-date medical information from anywhere in the world by you, your loved ones, emergency rooms, and providers of your care whom you authorize.
- Work with you and your physician to update the information periodically to ensure your record is always current.
- Work with you to maintain the reliability of the information in your record so that your physicians, hospitals, emergency rooms and other providers have confidence in the quality of the information in the record.
- Review the huge and often complicated records maintained by healthcare providers and summarize them to only the most important highlights of your medical history and care. This allows emergency personnel to quickly assess your history, especially if you are unable to tell them yourself.
- Maintain the strictest of confidentiality of your information including information security and HIPAA requirements.
