Through our partner, your Ho‘okele
Personal Health Record gives you access
to the Healthy Living Center where you can
find answers to your questions and information,
videos, and slide shows.
Ho'okele's Health Navigators will
make it easy and convenient for you to set
up and maintain your personal health record.
We will:
- Request a copy of your records from your doctors;
- Review the information and, if necessary, discuss it with your doctor;
- Enter your data into your Ho‘okele Personal Health Record for you;
- Assist you in completing the MediRisk health assessment questionnaires;
- Evaluate and explain your risk factors that resulted from the questionnaire;
- Review with you the MediKard emergency card;
- Train you how to use the robust features of MediManager; MediTracker; MediKabinet and the many other functions of your personal health record;
- Be available to answer your questions via MediMail;
- Keep your information up to date so that it is accurate in an emergency through quarterly verifications with you;
- Work with you to manage all the details of keeping your clinical information current so that you don't have to worry about it.
Your Ho‘okele Personal Health Record includes:
